On March 25, 2018, Service Ontario indroduced the "birth certificate with parental informaiton" into its public-facing Newborn Registration Service and

online certificate application system.  Ontario now offers two birth certificates: a birth certificate and a birth certificate with parental informaiton. The term "short form" and "long form" are no longer used.

The birth certificate with parental ifnormation is the recommended product of choice for neborns or children under 16 years of age . It can be used for the same identification purposes as a birth certificate, and also for:

  • A child's general passport
  • A child's health insurance
  • Certain immigration or citizenship applications
